Unlike traditional health maintenance organizations (HMOs) that limit members to receiving care from a specific medical group, with Vivity you are never restricted. You can choose which medical group you want for your primary care. You can always ask your doctor to refer you to any specialty in our network.

A ground-breaking HMO health
plan with simple copays and
no deductibles*
When you see one of the doctors in the Vivity network, you will know the cost, if any, ahead of time. In addition, everyone pays a copay through a flat-fee system. This helps make payments more predictable. Once you pay your copay for your care, Vivity pays the rest. There are no deductibles or hidden costs, which means no surprises*.

Members have
choices
You have your choice of primary care physician (PCP) to serve as your main doctor, but you are not restricted to a particular medical group. To make your healthcare journey simpler, Vivity allows you to ask your PCP for a referral to any specialty in the network, too.
You can also self-refer to go straight to some specialties (like allergy) without waiting for a referral from your doctor. Vivity is here to save you time and put you in control of whom you see.

Members have access to care, day or night
Many network doctors offer evening and weekend office hours for convenience. Registered nurses are available by phone for support and answers to health questions through 24/7 NurseLine at 1-800-700-0197.
For added convenience, virtual visits with a PCP are available by using each health system’s telehealth option. You can have video visits with a doctor 24/7 through LiveHealth Online for the same copay as an office visit. You can also access doctors on LiveHealth Online — or receive behavioral healthcare through LiveHealth online or select a behavioral specialist — by using a smartphone, tablet, or computer.

The free mobile app empowers you through features such as tap to call, access to digital ID cards and coverage information, telehealth visits, online health assessments, and coaching and wellness messages. You can also access your doctor’s patient website and find urgent care centers in the network.
